We provide high-quality support to adults with a learning disability and other complex needs by offering individualised care packages that incorporate social, vocational, leisure and educational activities and increase their presence in the community. 

Activity plans are tailored to each person's aspirations, needs and interests and are designed to help individuals to pursue a more independent lifestyle. All activities are fully risk assessed. A dedicated group of key workers are available to work with each individual to ensure a consistent approach with clear support plans in place.

We can provide services for people with varying support needs, including physical disabilities, sensory impairments, communication difficulties, cerebral palsy, acquired brain injury, epilepsy, pica and Down’s syndrome. We will provide the necessary staffing ratio for each individual.

We aim to:
• Provide work opportunities – paid and unpaid
• Teach enabling skills so that people gain the confidence to use public transport and local facilities
• Promote future goals and independence
• Support people to develop relationships
• Use local facilities to promote education
• Empower people to look after their own health needs by using local primary care facilities

We encourage the use of public transport wherever possible, but we can also offer transport from people's homes to activities. People will be accompanied on the journey by the same staff who support them at the activities to give the individual consistency and stability.

Our office is based in Uttoxeter and offers facilities for personal care needs, as well as a base to which people can return in an emergency or for refreshments. We do not use the base as a regular venue for activities as our aim is to provide a service in line with Valuing People and the modernisation of day services. We also have a separate training flat in the town where we can assess and teach independent living skills.

When an individual is referred to us, the manager and deputy managers will assess the referral and liaise with the relevant multi-disciplinary team to identify the level of support required. The assessment process will involve gathering information from the current service provider and family carers, as well as getting to know the individual.

Each person will have a thorough transition when they will be introduced to their key working team.

We would encourage individuals and their parents and carers to visit us to view the facilities and discuss the community network systems.
